The booklet aims to:
- to unify medical-sports documentation in all sports disciplines,
- to increase medical-sports control of athletes actively practicing sports for health reasons and to raise the level of performance
. The booklet is a document authorizing the athlete to train, training camps and participate in competitions
The booklet should be carried by the athlete
The medical part of the booklet is filled out only by the doctor certifying the ability to practice sports and participate in competitions, and the sports part is filled out by the coach
The basis and scope of the Seller's liability towards the Customer, if the sold Product has a physical or legal defect (warranty), are defined by generally applicable laws, in particular in the Civil Code (in particular, Articles 556-576 of the Civil Code).
